I'm a liver transplant recipient who looked at post-discharge care and saw a systemic gap. 15,000+ organ transplants in India annually. Millions of oncology follow-ups. Readmission rates of 35–40%. The signals that predict deterioration exist — there is no infrastructure to capture, score and act on them in time. I'm building that infrastructure. Ph (Internal codename) is a clinical decision layer sitting between patient-generated health data and clinical action — combining daily longitudinal data acquisition, a proprietary multi-dimensional AI scoring engine and real-time risk stratification for clinical teams. The goal is earlier intervention, alternative care pathways and measurably better outcomes at scale. **The core technology is patent pending.** The proprietary scoring engine — combining multi-dimensional vital threshold analysis, multi-system clinical combination pattern detection and short and medium-term predictive trend modelling — represents a novel approach to automated clinical risk stratification in post-acute care. It is not a rules engine. It is encoded clinical reasoning, built with and validated by a transplant surgeon, running continuously on every patient every day. My co-founder is a transplant surgeon — the same surgeon who operated on me. He owns the clinical protocol. I own the platform. On the technical side, I design and build full-stack AI systems — FastAPI backends, React frontends, multi-layer scoring engines, WhatsApp-integrated data pipelines and agentic AI workflows. I've built Ph entirely using AI — my engineering team, product team, QA and review layer are all AI agents. No traditional tech team.
